Please read these Terms of Service (“Terms”) carefully before using the website located at ascenttrt.com (the “Site”) or enrolling in services offered by Next Phase Medical PLLC, doing business as Ascent TRT & Weight Loss (“Company,” “we,” “us,” or “our”). By accessing or using the Site or our services, you agree to be bound by these Terms. If you do not agree, please do not use the Site or enroll in our services.
Acceptance of Terms
By accessing this Site or enrolling in services, you represent that you are at least 18 years of age, have read and understood these Terms, and agree to be bound by them. These Terms constitute a legally binding agreement between you and Next Phase Medical PLLC.

The content on this website is for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always seek the advice of a qualified healthcare provider with any questions you may have regarding a medical condition.
Use of this Site does not create a provider-patient relationship. A provider-patient relationship is only established upon completion of a formal consultation and intake process with a licensed provider at Ascent TRT & Weight Loss. Results are not guaranteed and there is no promise of cure.

Telehealth Services
Ascent TRT & Weight Loss provides telehealth services to patients in the state of Texas. Our services are available only to individuals who are physically located in Texas at the time of their consultation or treatment. Services are provided by licensed nurse practitioners and/or physicians operating within their scope of practice under applicable Texas law.
Telehealth services have inherent limitations compared to in-person care, including limitations on physical examination. By using our services, you acknowledge and accept these limitations. Telehealth services offered through this practice are not emergency services. In the event of a medical emergency, call 911 or go to the nearest emergency department. We reserve the right to decline to provide services to any individual at our clinical discretion.
Compounded Medications
Ascent TRT & Weight Loss may prescribe compounded medications as part of a supervised clinical program. Compounded medications are not reviewed or approved by the U.S. Food and Drug Administration (FDA) for safety, effectiveness, or quality. Medications are prescribed only after a comprehensive evaluation by a licensed provider and when deemed medically appropriate. Individual results vary and are not guaranteed.
Pricing, Billing, and Recurring Charges
Program Fees
All program fees are clearly stated on our website. Fees are subject to change with 30 days’ written notice via email or text. Continued use of services after such notice constitutes agreement to the new rate. Program fees cover provider services, ongoing clinical supervision, and where applicable, medication as a component of the program. We do not sell medications directly.
Recurring Billing Authorization
By enrolling in a monthly or quarterly plan, you authorize Next Phase Medical PLLC to automatically charge your credit card on file for the agreed-upon program fee on a recurring basis (monthly or quarterly, depending on your selection). Your subscription will automatically renew at the end of each billing cycle unless you cancel in accordance with our cancellation policy below.
Credit Card on File
You authorize Next Phase Medical PLLC to store your credit card information securely on file and to charge it for:
- Recurring subscription fees as agreed upon during enrollment
- Ancillary services or products ordered outside your subscription
- Missed appointment fees as described below
- Any past-due balances outstanding more than 60 days
You are responsible for keeping your card information current. If your card expires or changes, you must update it promptly.
Chargeback Waiver
By enrolling in services, you waive your right to dispute valid charges with your bank or card issuer (chargebacks) so long as those charges correspond to the terms outlined in this agreement. We encourage you to contact us directly at info@ascenttrt.com to resolve any billing concerns before initiating a dispute.
Failed Payments
If your automatic payment declines, we will attempt to contact you to update your payment method. If payment is not resolved within 7 days, your medical services will be suspended and no further medication shipments will be released until your account is current. If your account remains past due for more than 60 days, it may be referred to a collection agency. You agree to pay all collection fees and legal costs incurred.
Cancellation Policy
Monthly Plans
You must provide 30 days’ written notice to cancel a monthly subscription. Written notice must be sent to info@ascenttrt.com. This ensures that no new medication orders are placed on your behalf. If notice is given less than 30 days before your next billing date, that final payment will be processed and one final shipment will be fulfilled.
Quarterly and Pre-Paid Plans
Quarterly and pre-paid plans offer a discount in exchange for a commitment. Once a 3-month supply of medication has been ordered or dispensed, no refunds will be issued for the remaining duration of that quarter.
No Refunds on Medications
Federal regulations prevent the return of prescription medications once they have left the pharmacy. All sales of medications and compounded preparations are FINAL and NON-REFUNDABLE once the order has been submitted to the pharmacy, even if you decide to stop treatment early.
If you have concerns about a medication order, please contact us before shipment is initiated. We will make every reasonable effort to accommodate requests made prior to order submission.
Additional Fees
The following additional fees may apply to your account:
- Missed Appointment: $25.00. A minimum of 48 hours’ notice is required to cancel or reschedule an appointment without charge. Failure to provide sufficient notice or failure to appear will result in a $25.00 fee charged to your card on file.
- Returned Check: $20.00 for any check returned by the bank.
- Medical Records Copies: $0.50 per page for copies of lab work, chart notes, imaging, and invoices, except when requested at the time of a visit.
- Administrative Forms: $25.00 per document for completion of special insurance forms, workplace documentation, or letters of medical necessity. Payment is required in advance.
Insurance
Next Phase Medical PLLC does not file for insurance reimbursement. You are responsible for all fees at the time of service. Upon request, we can provide a receipt or invoice for your records. We do not automatically generate Superbills for recurring subscription fees, as many insurers do not reimburse for membership-based medical care.
Supplement Disclaimer
Many supplements, vitamins, medical grade foods, nutritional powders, botanicals, and homeopathic remedies recommended or available through this practice have not been evaluated by the U.S. Food and Drug Administration (FDA). These products are not intended to diagnose, treat, cure, or prevent any disease. All supplement sales are final. No refunds, credits, or exchanges are allowed on any supplement. Supplements will not be held, picked up, or shipped without prior payment.
SMS Communications
Message frequency varies. By providing your mobile phone number and requesting services through this Site, you consent to receive SMS messages from Next Phase Medical PLLC d/b/a Ascent TRT & Weight Loss related to your care, appointments, and account. Message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for assistance. Your phone number will not be shared with third parties for marketing or promotional purposes. Email Communications
By providing your email address, you consent to receive service-related communications from us. You may also receive promotional emails about our services. You may opt out of promotional emails at any time by clicking the unsubscribe link in any email. Opting out of promotional emails does not affect transactional or care-related communications.
Intellectual Property
All content on this Site, including text, graphics, logos, images, and software, is the property of Next Phase Medical PLLC or its content suppliers and is protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content on this Site without our express written permission.
User Conduct
You agree not to:
- Use this Site for any unlawful purpose
- Attempt to gain unauthorized access to any portion of the Site or its related systems
- Submit false or misleading information
- Interfere with the proper functioning of the Site
- Use the Site to transmit spam or unsolicited communications
Disclaimer of Warranties
This Site and its content are provided on an “as is” and “as available” basis without warranties of any kind, either express or implied. Limitation of Liability
