Please read these Terms of Service (“Terms”) carefully before using the website located at ascenttrt.com (the “Site”) or enrolling in services offered by Next Phase Medical PLLC, doing business as Ascent TRT & Weight Loss (“Company,” “we,” “us,” or “our”). By accessing or using the Site or our services, you agree to be bound by these Terms. If you do not agree, please do not use the Site or enroll in our services.

Acceptance of Terms

By accessing this Site or enrolling in services, you represent that you are at least 18 years of age, have read and understood these Terms, and agree to be bound by them. These Terms constitute a legally binding agreement between you and Next Phase Medical PLLC.

Relationship to Your Signed Agreements

These Terms govern your use of this website. The specific terms of your care are set out in the documents you sign when you become a patient, which include our Consent and Practice Policies, our Financial Agreement, and any treatment-specific consent that applies to you.

Membership plans, prepaid packages, billing, cancellation, reinstatement, and termination of the patient-provider relationship are governed by the Financial Agreement and Consent and Practice Policies you sign at enrollment. In the event of any conflict between these Terms and your signed agreements, your signed agreements control.

Not Medical Advice

The content on this website is for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always seek the advice of a qualified healthcare provider with any questions you may have regarding a medical condition.

Use of this Site does not create a provider-patient relationship. A provider-patient relationship is established only upon completion of a formal consultation and intake process with a licensed provider at Ascent TRT & Weight Loss. Results are not guaranteed and there is no promise of cure.

Telehealth Services

Ascent TRT & Weight Loss provides telehealth services to patients in the state of Texas. Our services are available only to individuals who are physically located in Texas at the time of their consultation or treatment. Services are provided by licensed nurse practitioners and physicians operating within their scope of practice under applicable Texas law.

Telehealth services have inherent limitations compared to in-person care, including limitations on physical examination. By using our services, you acknowledge and accept these limitations.

Telehealth services offered through this practice are not emergency services. We do not provide 24/7 or after-hours care. In the event of a medical emergency, call 911 or go to the nearest emergency department. We reserve the right to decline to provide services to any individual at our clinical discretion.

Prescription and Compounded Medications

Medications are prescribed only after evaluation by a licensed provider and only when clinically appropriate. Submitting a request through this Site does not guarantee that any medication will be prescribed.

Some medications may be prescribed off-label, meaning for a purpose other than the one for which the FDA approved them. Off-label prescribing is legal and common in medical practice, and your provider will discuss the specific reason a medication is being prescribed for you.

We may prescribe compounded medications as part of a supervised clinical program. Compounded medications are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness, or quality before they are dispensed. Compounded medications are prepared by licensed compounding pharmacies. Individual results vary and are not guaranteed.

Specific risks, benefits, and alternatives for any medication prescribed to you are covered in the treatment-specific consent you review and sign with your provider.

Pricing and Payment

Current pricing for our plans is published on this Site. We do not file insurance, and you are responsible for all fees.

Billing, recurring charges, cancellation, refunds, and all other financial terms are governed by the Financial Agreement you sign at enrollment. Prices are subject to change with notice as described in that agreement.

Communications

Email

By providing your email address, you consent to receive service-related communications from us. You may also receive promotional emails about our services. You may opt out of promotional emails at any time by clicking the unsubscribe link in any email. Opting out of promotional emails does not affect transactional or care-related communications.

Text Messages

By providing your mobile phone number and requesting services through this Site, you consent to receive SMS messages from Next Phase Medical PLLC d/b/a Ascent TRT & Weight Loss related to your care, appointments, and account. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for assistance. Your phone number will not be shared with third parties for marketing or promotional purposes. Carriers are not liable for delayed or undelivered messages. See our Privacy Policy for full details.

Intellectual Property

All content on this Site, including text, graphics, logos, images, and software, is the property of Next Phase Medical PLLC or its content suppliers and is protected by applicable intellectual property laws.
